AUGUSTA (AP) — The Maine veterans’ hospital in Augusta has launched an investigation after patient records were dumped unshredded into the trash.

A spokesman for the Maine VA Medical Center says the documents never left the grounds and no personal information was disclosed.

A bag containing the documents was discovered in a trash bin on April 11.

An employee told the privacy officer at the medical center and the documents were collected.
