SMART news
SMART Child and Family Services has a new program called Children’s Targeted Case Management Services. With this expansion of services, they can now provide case management services to children ages 4-18 with a mental health diagnosis (PTSD, depression, anxiety, PDD, etc.) who have need of a supportive professional. This professional case manager will help parents and children locate appropriate resources and assist in gaining access to needed services.
Case managers work with parents and children to make an assessment of needs, develop and implement an individual support plan, link and coordinate services, among other things.
To make a referral to any of SMART’s programs, call 893-0386.
