It’s that time of year again to think about applying for fuel assistance. A walk-in site will be open at Our Lady of Perpetual Help Church from 8:30 a.m. to noon and 1 to 3:30 p.m. on Tuesday, Sept. 13 for priority clients only including people age 60 and over and households with children under 24 months. All others can come to the church Nov. 14 through 17 at the same times for help with filing claims.
For those clients who are homebound, contact the office at 854-2988. For those who are self-employed, please call 854-2988 ext. 32 to schedule and appointment.
On the day you apply, you must bring the following information with you:
•Proof of income for all household members, for either the last 3 months or 12 months prior to the month of your appointment.
•Date of birth and Social Security numbers for all household members.
•A current electric bill, telephone bill, and account number for your fuel company.
•If you’re a renter bring your landlord’s name, address, phone number and a current rent receipt.
