BRUNSWICK — CHANS Home Health Care is offering Telehealth services that allows patients to monitor their health at home, providing faster responses to changes in health that could have otherwise led to hospitalization. This new technology, primarily for patients with diagnoses of Congestive Heart Failure or Chronic Obstructive Pulmonary Disease, helps individuals manage chronic diseases and reduce the overall costs of healthcare by reducing Emergency Department visits and hospital readmissions. The technology provides daily in home assessment through a Samsung 4G tablet. Telehealth is provided as part of CHANS Home Health Care nursing services and is available at no additional charge to the patient or insurance.
