BATH — The Salvation Army is taking applications for holiday assistance through Dec. 4.
Interviews are done Monday, Wednesday and Thursday from 9 a.m. to noon. Individuals should sign up early as time is limited during the Christmas Season.
To request assistance, it is important to bring the following items: form of identification for all members in the household; documentation of all income in the household; documentation of all expenses; and children’s clothing sizes and wish list.
The following are accepted forms of ID: Social Security cards, MaineCare cards, driver’s license or state ID.
The following are accepted forms to prove income: bank statements, Social Security letter, pay stubs for 30 days, Department of Health and Human Services review letters or print out of form at www.Pinetree.com for T.A.N.F or SNAP benefits.
The following are accepted to prove expenses: bills or receipts for rent or mortgages, utilities bills, car insurance, car payments and phone, cable and credit cards statements.
